Please call our office at 913-287-7800 to schedule your New Patient visit.

Please come 20 minutes early, on your first visit, to allow extra time to complete the "new patient" paperwork.  You will receive a New Patient Packet at this visit.

Patient information needed every visit

• Please bring picture ID
• Please bring current medication bottles
• Copays are due at the time of service
