What To Expect From Your Care Team
Team-based Care
- One of us will remain as your Primary Care clinician. However, we will have a team of nurses and support staff to work with you to meet all of your health care needs.
- You may be asked by your provider to complete a Health Risk Assessment and participate in developing a care plan, which includes individualized patient goals during your visit and helps organize the right team members to be involved with your care.
Improved Health Access and Communication
- For urgent care issues during working hours, your Primary Care clinician (or another one of our team members) will attempt to see you on the very day that you have an urgent health care need. You will simply need to call the main office number during working hours to schedule a same-day appointment with us. Many urgent health care needs can be handled by your Medical Home team. You will then avoid having a prolonged and expensive visit to the Emergency Room.
- Better communication and cooperation between patients and their care team improves the likelihood that you will be in a better position to manage your health.
Insurance Requirements:
- You don’t need insurance to be seen at our clinic.
- If you do have insurance, please bring your information with you.
- United Medical Group wants to partner with our patients to provide up-to-date health information and recommends that you participate in self-management and support activities offered in the community. As always, we provide direct communication at your visit, via phone or the patient portal.
